The Cook Home

Licensed for 18 years and above. One bed/Room is available. It has been open since June 26, 2018. It is a 5-Bed room home, licensed for 4-bed rooms with the 5th one being the office and medication storage. It has got a wheelchair ramp, and bathroom handrails. It is strategically located, and very close to a city bus stop, a gas station, a lot of shops such as: Cub foods, Aldi, Dollar tree, as well as fast food restaurants, such as: McDonald’s, Dairy Queen (DQ), Subway, and many others and banks too, such as: Wells Fargo and US Banks. Lake Phalen is also within walking distance. We accept both CADI and Elderly waiver. Some of our current clients are independent, some are semi-independent, and some are full service (Fully dependent). We take all kinds of clients/residents.

We are not GRH certified yet, although we are working on it, for that reason we negotiate fairly with our potential residents’ care team and settle for what the client/resident can pay.
