Perfection Health Care Services, Inc. is in Saint Paul, Minnesota. We have two locations:

A-1225 Cook Avenue East, Saint Paul, MN 55106 (Licensed for 18 years and above). One bed/Room is available.

It has been open since June 26, 2018. It is a 5-bedroom home, licensed for 4-bed rooms, with the 5th one being the office and medication storage. It has got a wheelchair ramp and bathroom handrails. It is strategically located and very close to a city bus stop, a gas station, a lot of shops such as Cub Foods, Aldi, and Dollar Tree, as well as fast food restaurants, such as McDonald’s, Dairy Queen (DQ), Subway, and many others and banks too, such as Wells Fargo and US Banks. Lake Phalen is also within walking distance. We accept both CADI and Elderly waivers. Some of our current clients are independent, some are semi-independent, and some are full-service (Fully dependent). We take all kinds of clients/residents.

We are not GRH certified yet, although we are working on it. For that reason, we negotiate fairly with our potential residents’ care team and settle for what the client/resident can pay.

B- 2092 Oakridge St., Saint Paul, MN 55119 (Licensed for 55 years old and above). Three beds/Rooms are available.

The Oakridge house is a 6-bedroom brand new 2587 Sqft house built in 2016. It is a 6-bedroom home, licensed for 5 rooms, with the 6th one being the office and medication storage. It has a wheelchair ramp and wooden floors, which is great for clients in wheelchairs. All the wheelchair-accessible rooms are on the main floor. It is fully equipped and newly furnished. This is a lovely one-story home tucked away in the quiet, wooded Highland Hills neighborhood of St Paul. It is truly one of a kind, designed to accommodate many kinds of households. The main level offers everything you need, with 3 bedrooms, including an owner’s suite with a full bath, an additional full bath & a laundry room. The lower level is fully finished, with egress in all the rooms. It has another 3 bedrooms, a 2nd kitchen & a full bathroom. The garage is huge! Listed as 2 stalls but could likely fit more. The upstairs is finished with a new carpet and just needs heat for a separate office or amusement room. Built in 2016, everything is nearly new in this beautiful and well-kept home!

What do we provide?

We provide waiver services in Minnesota in residential four to five-bedroom single-family homes. There is no sharing of rooms. Every resident gets their own room with a queen-sized bed with brand-new bed sheets, a dresser, a stand, a huge closet, and a Roku TV connected to our high-speed Comcast Business Wi-Fi internet.

We provide both Licensed Practical Nursing (LPN) and Registered Nursing (RN) services. We provide medication management, medication administration, medication set-ups, and medication reminders. We make sure that we order monthly medication in advance.

We deal with Geritom Pharmacy for all medications which are delivered to our doorsteps. We check both resident medications and medication supplies daily. We keep resident medications in a secured and locked steel cabinet inside a secured and locked office. In addition to that, we put a metal box in the metal cabinet, which is also inside the office, for all controlled medications (Narcotics). We keep all laundry detergents inside the office for residents’ safety. We have a 24/7 awake staff and ADT security systems at all our doors for the safety and security of our residents and peace of mind for their caregivers, loved ones, and other responsible parties. We make all our residents’ medical appointments, drive them there, and be there with them and bring them back. We provide all our residents with three meals a day, plus two snacks. We do all the daily housekeeping. The laundry is done twice a week, one during the week and the other on the weekends, and as needed, too. We provide Xfinity Cable TV on the main common areas of every floor and Roku TV in every resident’s room. All our homes have got wheelchair ramps and are wheelchair accessible.

What are the documents we need?

When someone is looking for housing for a client(s), we ask them to please send us the client(s) documents, such as The MN Choice, The CSSP, the CSP, the face sheet, the hospital stay summary, and so on. We then go through them and respond within the same day, and if we decide that we can serve the client well, we send our yes! Response with a 6790.

We will also need the contact details of the resident’s rep payee for the Room and Board and if the resident has any spenddowns, co-pays, and so on.

We would like to be the facility that comes to your mind first whenever you are in search of housing for your client(s). We will be delighted to serve your client(s) and that of your colleagues to the best they all deserve.

Note: As a principle, we allow residents to move in if and only if we receive an approved/guaranteed 24/7 Customized Living Daily Rate, a “Service Agreement Letter” is in the Minutes System (MN-ITS), and a Rep Payee is in place with agreed Room and Board (R&B) fees.

In short, as soon as everything is approved from all sides involved, the resident can move in.
